At SDG, care isn’t one-size-fits-all or one-and-done. We offer a layered ecosystem of support designed to meet you where you are — and evolve as your needs do.
You don’t need to choose everything. We’ll help guide you to what makes sense — when it makes sense.
CARE & SUPPORT
You may move through one or more of these layers — or stay where you are.
Care is always guided and never required.
Personalized 1:1 Nutrition Care (Insurance-Based)
Optional Community Support & Resources
Short-Term Programs for Specific Goals
Specialized & Medical Nutrition Support
This is where most people begin. Foundation care includes:
+ Individual nutrition visits with a registered dietitian
+ Personalized medical nutrition therapy with plan
+ Goal-setting, problem-solving, and plan adjustments
+ Follow-up visits based on medical need
This layer focuses on clarity, safety, and clinical care.
Most clients start here.
FOUNDATION
Some people benefit from staying connected between visits.
Connection includes:
+ A free community space
+ A free meal planning club with recipes and educational content
+ Free group challenges and events
+ Free meetups and opportunities to connect with others, such as walking groups, coffee meet-ups, and more
Connection is always optional and pressure-free.
It’s here if connection feels helpful — and okay if it doesn’t.
CONNECTION
Some needs require more specialized care.
Precision support may include:
+ Advanced gut health testing and nutrition
+ Hormone Testing and Hormone Replacement Therpay
+ Metabolic Support such as Low-Dose GLP-1 Therapy
+ Micronutrient Testing
+ Other Specialized nutrition services
This level is offered when clinically appropriate and guided by your care team.
PRECISION
At certain points, more focused support may be helpful.
Focus programs are:
+ Time-limited
+ Goal-specific
+ Offered intentionally, when appropriate
Examples include accountability sprints or data-informed programs such as continuous glucose monitoring.
These are never required and are introduced thoughtfully.
We’ll help determine if and when Focus makes sense.
