+ Personalized medical nutrition therapy with plan
+ Goal-setting, problem-solving, and plan adjustments
+ Follow-up visits based on medical need

Connection includes:

Momentum is our optional membership for people who want continued support between visits.

Momentum is designed to support progress — not perfection. Momentum focuses on:

Ongoing Support for Consistency & Change (Optional)

+ Mindset, nervous system, and emotional health
+ Sustainable nutrition education
+ Guidance during plateaus or setbacks

Momentum is not required for care — and can be joined or paused at any time.

FOCUS

At certain points, more focused support may be helpful.

Focus programs are:

+ Time-limited
+ Goal-specific
+ Offered intentionally, when appropriate

Examples include accountability sprints or data-informed programs such as continuous glucose monitoring.

These are never required and are introduced thoughtfully.

We’ll help determine if and when Focus makes sense.
